CONSENT, RELEASE OF LIABILITY, AND INDEMNITY AGREEMENT

I (“Releasor”), the undersigned parent or guardian having legal custody of the student listed above (“Child”), execute this CONSENT, RELEASE OF LIABILITY, AND INDEMNITY AGREEMENT (the “Release”) on the date indicated below for and on behalf of my Child, and me as parent/guardian, in favor of the Mayfair Church of Christ (“Mayfair”) and the respective elders, employees, ministers, officers, interns, members, trustees, and agents of such organization (collectively, the “Released Parties”).
I acknowledge that my Child desires to participate in the Mayfair Youth Program and as a participant may engage in a variety of events (the “Events”) carried out during the period of August 16, 2026 - August 1, 2027. The Events will be part of the Mayfair Youth Ministry (the “MYM”) carried out by and under the direction of Mayfair and associated individuals assigned responsibilities in connection with the MYM. The Events may involve a number of activities (the “Activities”), such as traveling to and from the Mayfair facility and various off-site locations, engaging in active games and sporting/recreational activities (such as hiking, volleyball, swimming, etc.), visiting theme parks and other outdoor or indoor attractions, staying overnight at an off-site location (a hotel/motel, cabin, camp, etc) with other MYM participants and supervising Mayfair adults, interacting with MYM participants and other individuals, attending worship services, teaching and assisting with recreational children’s and youth activities, engaging in service projects, and other activities reasonably related to and/or connected with the Events.
I further acknowledge the personal, educational, recreational, and spiritual benefits that my Child will receive as a result of participation in such Events.
I hereby freely, voluntarily, and without duress execute this Release and agree to the following:
1. Consent to Participation - I do agree and consent to my Child’s participation in the Events and Activities described above. I further state that, to the best of my knowledge, my Child is able, in terms of physical capabilities and developmental maturity, to participate in the Events and Activities. Any limitation(s) on my Child’s ability to participate will be disclosed to the Released Parties below.
2. Assumption of Risk - I am aware of the risks and hazards to my Child accompanying his/her participation in the above-described Events and Activities, including the risks ordinarily attendant to travel by air, bus, and automobile, and all other risks of personal or bodily injury (including death) normally incident to, associated with, or reasonably foreseeable from the Events and Activities. In consideration of the benefits to my Child recited above, and other good and valuable consideration, I do expressly, knowingly, and willingly assume, for my Child, such risks and hazards.
3. Release and Waiver - In further consideration such benefits, and other good and valuable consideration, I do hereby, for myself and my Child, release and forever discharge, indemnify and hold harmless, and covenant not to sue the Released Parties from and concerning any and all causes of action, claims, demands, or liabilities of whatever kind or nature, either in law or equity, for personal or bodily injury (including any resulting in death) or other damages or losses, from whatever cause or by whomever caused, that may arise out of, occur during, result from, or be connected in any way with my Child’s participation in the Events and Activities, including actions taken under paragraph 4 below to provide or arrange, or to not provide or arrange, for medical services, procedures, and/or treatment. It is my intention that the negligence of the Released Parties or any of them shall be within the scope of this Release and that only wanton or willful misconduct by such parties is excepted therefrom.
4. Consent to Medical Treatment -
- I do agree that, at any time during my Child’s participation in an Event or Activity, any adult person acting on behalf of Mayfair is authorized to provide reasonable first aid measures, to arrange for medical services, procedures, and treatment by a medical provider properly licensed in the United States and/ or at a healthcare facility properly licensed in the United States, and to take other reasonable and necessary actions with respect to my Child in the event of his/her injury or illness. Such authorization includes signing a medical authorization form.
- I do further authorize such person(s) to enter into any agreement deemed necessary or appropriate, in the sole discretion of such person(s), for the health and well being of my Child, including an agreement to pay for the medical services/procedures/treatment provided to my Child.
- I assume full and sole responsibility for and agree to pay all bills for all medical services/procedures/treatment provided to my Child. I understand that the Released Parties will not have any obligation to pay such bills.
- I agree to provide all relevant information regarding my Child’s health, allergies, medication, physical or other limitations, etc. in the space provided at the end of this Release. If there is any material change in such information occurring in the future, I will notify a MYM minister or employee of such change.
5. Insurance - I understand that, unless otherwise agreed to by the Released Parties in writing, the Released Parties shall have no obligation to and will not carry or maintain health, medical, disability, or life insurance coverage for my Child with respect to his/her participation in any Events or Activities. I acknowledge that the responsibility to carry and maintain any such insurance falls on me alone, and I hereby certify that such insurance coverage for my Child is and will be in effect for the period of time encompassed by this Release.
6. Photographic/Recording Release - I do grant and convey unto the Released Parties all right, title, and interest in and to any photographic images and video or audio recordings made by the Released Parties with respect to my Child during any Event or Activity. This grant includes, but is not limited to, any royalties, proceeds, or other benefits derived from such photographic images or recordings.
I expressly agree that this Release shall be governed by and interpreted in accordance with the laws of the State of Alabama and is intended to be as broad and inclusive as permitted by such laws. I further agree that, in the event any clause or provision of this Release shall be held to be invalid by a court of competent jurisdiction, the invalidity of such clause or provision shall not otherwise affect the remaining provisions of this Release, which shall continue to be valid and enforceable. This Release shall be valid until revoked by me and my revocation is communicated to a MYM minister or employee.
