My Statement of Commitment (Sign Below)
I hereby affirm the following:
I will participate in the preparatory team meetings either in-person, via Zoom, or by watching an online recording or reading an electronic summary of the meeting (if made available to me).
I will give the best of myself during this trip, understanding that much is expected of each member of this mission team.
I understand that serving as a member of this mission team requires that I conduct myself as an Ambassador of Jesus Christ and His Church.
As an Ambassador, I understand that I am expected to conduct myself in a Christlike way, considering others more important than myself (Philippians 2:3-4), taking on the form of a servant (Philippians 2:5-7), and recognizing that all people are beloved of God (John 3:16), even on their worst days.
I commit myself to enduring personal inconveniences with Christlike patience for the sake of God's mission to reconcile lost people to Himself. I do this, remembering the sufferings of the first Christian missionaries who walked hundreds of miles, crouched in wave-tossed ships, went hungry, and even endured physical beatings for the sake of the gospel.
I realize that human anger does not produce the righteousness of God (James 1:20). Therefore, I commit myself to setting aside ALL displays of anger, bitterness, slander, and shouting (Ephesians 4:31) while serving as a member of this mission team.
I commit myself to making deliberate efforts to encourage and build up fellow team members and others (Ephesians 4:29) during this trip.
I agree to follow all rules and guidelines that are set forth by the Mayfair elders, trip leaders, and the Baja Missions Handbook. I understand that such rules are made for my personal safety and for accomplishing mission objectives.
I agree to transport ministry supplies such as Bible class materials, outreach fliers, vitamins, over-the-counter medicines, clinic supplies, benevolence supplies, and small items for construction projects in my personal luggage. I also understand that I will likely be asked to transport a second piece of checked luggage and that the luggage fees are my responsibility, whether paid from my own funds or by cash donations from others.
I understand that, upon notification that my application is accepted, I (or my parent or legal guardian) must promptly submit additional information, including but not limited to a liability waiver, a copy of my passport, a copy of my medical insurance card, and medical/dietary information to help assure my wellbeing on this trip.
Refunds. I understand that I may request a refund of my $100 deposit if I am not selected as a member of this mission team for any reason. I understand that, if I must cancel my plans to participate in this trip after being selected, trip leaders will make a good faith effort to refund as much of my fee as possible. I understand tha my initial $100 deposit may not be refundable after the first team meeting.
Payment instructions will be provided on screen and via email after submitting this form.
